Case Management / Early Intervention Services
Medical Case Management (case management) provides a range of client centered services to people living with HIV to ensure clients are linked with health care, health insurance, psychosocial and practical support services to reduce barriers to maintaining health care. Case Management helps clients to maintain healthy living and continue with medical treatment to stay healthy. We provide assistance for transportation issues, housing, nutritional needs, assistance with medical insurance or prescription coverage as well as many other facets of maintaining healthy living.
Through a relationship with your case manager individual needs are identified and plans of care developed. In partnership case managers and clients ensure barriers to HIV care are addressed. This includes individual assessments, and eligibility certifications for Tennessee Ryan White services.
Early Intervention Services (EIS) are designed to support individuals who are newly diagnosed with HIV, are pregnant, recently incarcerated or who have been out of HIV medical care for over a year. EIS support provides intensive support to identify and reduce barriers to accessing HIV medical care and are provided as part of a comprehensive case management team.
